Simple refreshes shouldn't be counted, and should be fairly easy to detect (just compare the current URI to the "Referer" header).

It would be even better if you could scan the users "recently viewed pages" history so as to only incremement the count once per session

That would instantly remove problems when people are using linear navigation to scroll through pages (scrollbar) and then deciding to scroll back a few pages. It would also remove incremements when comments/child pages are shown/hidden, etc.
